This form is used to establish an agreement between you (COTM Missions trip participant) and your church (Church on the Move) Please read the following points and if you agree and will comply please sign electronically at the end of the document. Understanding that failure to comply with these points will result in removal from the team. 1. I agree to set up my Public Profile on the "managed missions" program sent to me by COTM Missions in my approval email and will use that avenue to get donations or individually pay for trip costs. 2. I agree to pay a $300 deposit within 2 weeks of receiving my approval. 3. I agree to have 50% of my trip balance paid 3 months before trip departure. 4. I agree to have 100% paid 2 weeks before departure. 5. I agree to watch, from beginning to end, every pre-trip training video sent by COTM Missions. 6. I agree to make scheduled trip gatherings a top priority. *For example I will not schedule other events during these times, I will ask off of work, switch shifts, miss small group and or family functions to be at the team gathering. 7. I agree to be a team player that AVOIDS every opportunity of complaining, drama, bickering, gossip and cliques. 8. I agree that I will follow the instructions and vision laid out by trip leaders. 9. I agree to report to team leaders abusive behavior of any kind during the trip. 10. I agree that I will refrain from using alcohol or illegal drugs while on this trip. 11. I agree to complete any immunizations that might be required by governments to travel to that specific country. 12. I agree to have fun, be myself, and be ready to grow spiritually. *If you are under the age of 18, a parent or legal guardian must check 'agree' and sign below.

Participant warrants and agrees that he/she has read this Release, understands its terms, and has signed this Release by his/her own free will. Participant acknowledges that he/she will participate in the missions trip (“Missions Trip”) to be conducted by or in association with Church On The Move. Participant releases and agrees to hold the Church, and its directors, officers, employees, volunteers, and agents (collectively referred to as “the Church”) harmless from all liability resulting directly or indirectly from Participant’s participation in the Missions Trip, including but not limited to liability for harm to Participant or his/her personal property resulting from the Church’s negligence or from illness, disease, civil unrest, war, terrorism or transportation accident or failure. Participant, individually and on behalf of his/her heirs, personally assumes all risks and liabilities in connection with Participant’s participation in the Missions Trip and agrees to indemnify the Church against any liability which might be assessed against it as a direct or indirect result of Participant’s participation in the Missions Trip. Participant consents to the Church photographing and/or recording the Missions Trip and agrees that any photographs or recordings of Participant taken by the Church in association with the Missions Trip are the exclusive property of the Church for use by the Church as it sees fit. The Participant further consents to Church on the Move using photographs and recordings taken by Participant in association with the Missions Trip. Participant shall not be entitled to any compensation for such use. If Participant is injured during the Missions Trip and Participant or one of Participant’s family members is unable to authorize medical treatment, Participant hereby authorizes dental, medical, or surgical treatment, including but not limited to the administration of X-rays, anesthetic or anesthesia by any medical professional chosen by the Church. Participant understands and agrees that this consent is given to encourage the Church and said medical professional to exercise their best judgment as to such diagnosis or medical, dental, or surgical treatment. Participant personally assumes the duty of payment to any physician, dentist, surgeon, hospital, clinic, or ambulance service and releases the Church from any such duty of payment. *If you are under the age of 18, a parent or legal guardian must check 'agree' and sign below.
